Abstract
Retroperitoneal paragangliomas are rare neoplasm of a neural crest origin. They are developed at various body sites and most are located in the para-aortic space along the sympathetic chain. Pancreas, mesentery are very rare sites for paraganglioma. It is necessary to rule out the possibility of paragaglioma that is no characteristic symptom and radiologic finding. We experienced a case of paraganglioma misdiagnosed as pancreatic head cancer. A 58 year-old male admitted for a pancreas head mass detected by abdominal sonography. It showed a cystic mass stuck on the head of pancreas. The mass was located in retroperitoneal space and dissected from surrounding structure. Microscopically, the tumor cells were arranged in a Zellballen pattern. It was diagnosed as a cystic retroperitoneal paraganglioma.
